For regular admission, including indigent, indigenous people, PWD, and Iskolar ng Bayan, you may download this College Grades Form here. For Alternative Learning System (ALS) graduates, you may download this form. On the other hand, admission requirements, and procedures can be accessed here.
Once you have completely filled out the College Admission Grades Form, you may view the application for admission at https://dione.batstate-u.edu.ph/tao/#/application
